What Is Blepharoplasty Plus?

Blepharoplasty Plus is The Ezra Clinic’s advanced approach to eyelid rejuvenation. Rather than focusing solely on removing excess skin or under-eye bags, Blepharoplasty Plus combines eyelid surgery with complementary procedures such as fat repositioning, midface elevation, brow support and volume restoration where required. The goal is to create natural-looking, long-lasting results by addressing the wider anatomical changes that contribute to a tired appearance around the eyes.

Lower Eyelid Blepharoplasty – Before and After

This patient underwent treatment to address under-eye bags and excess upper eyelid skin, resulting in a refreshed yet natural-looking appearance. Her lower lid blepharoplasty was performed using the scarless transconjunctival approach, with careful fat re-draping to smooth the under-eye contours and create a seamless transition between the eyelid and cheek.

Lower Lid Blepharoplasty

This lady’s lower eyelid blepharoplasty was performed using a transconjunctival approach, meaning the entire procedure was carried out through an incision on the inner surface of the eyelid, leaving no visible external scar. Fat repositioning was central to the technique, reflecting a tissue-preservation philosophy that prioritises restoring volume and contour rather than simply removing fat.

Her six-month postoperative photographs demonstrate a marked improvement in the under-eye area, with smooth contours and a naturally rejuvenated appearance.

Bilateral Lower Lid Blepharoplasty With A Midface Lift

This patient presented with significant asymmetry of the lower lid bags, accompanied by pronounced hollowing beneath the eyes. To address these concerns, a tissue-preservation lower lid blepharoplasty was performed, focusing on lifting, tightening and repositioning the existing tissues rather than removing them.

By carefully restoring volume and improving lower eyelid contours, the procedure created a smoother, more balanced appearance while maintaining a natural look. The postoperative photographs demonstrate a significant improvement in under-eye symmetry and overall facial harmony.

Blepharoplasty Under Eye – Before and After

This patient was concerned by persistent under-eye bags and puffiness, which often gave her a tired appearance. To address these concerns, an external approach lower lid blepharoplasty was performed using a tissue-preservation technique.

The procedure involved careful fat repositioning to restore volume in the hollow areas beneath the eyes and improve the transition between the lower eyelid and cheek. The postoperative photographs, taken six weeks after surgery, show a smoother under-eye contour and a fresher, more rested appearance.

Bilateral Upper and Lower Lid Blepharoplasty Plus

This patient underwent a lower lid blepharoplasty with fat repositioning, midface elevation and direct brow lift to address both upper eyelid heaviness and lower lid bags. The combination of procedures opened the upper eyelid area while creating a smoother, more youthful transition between the lower eyelid and cheek.

The postoperative photographs demonstrate a significant improvement in both the upper and lower eyelids, resulting in a refreshed appearance while maintaining the patient’s natural facial features.

Upper and Lower Eyelid Blepharoplasty, Midface Lift & Fat Transfer

This patient had concerns about overhanging upper lid skin and lower lid bags. Examination revealed excess skin in both areas, along with orbital fat herniation. Midface descent had also caused a separation between the eyelid and cheek. The goal was a natural, refreshed look without drastic changes.

Lower eyelid blepharoplasty addressed the under-eye bags, while upper blepharoplasty eliminated the upper lid hooding. A midface lift restored the cheek, creating a smoother transition between the eyelid and cheek. Fat transfer re-volumised the cheeks, enhancing the natural contour and improving skin quality.

The patient was delighted with the results, a refreshed, natural appearance that removed the tired look while preserving her natural features, as seen in the before and after images.

Upper and Lower Blepharoplasty with Midface Lift

This gentleman was bothered by lower lid bags which had been present for many years and getting progressively worse.  He had quite significant midface descent illustrated by the descent of the cheeks and drawn appearance.

There is also some mild overhang of the upper eyelid skin. It is essential that male eyelid surgery ensures that the distinctive gender specific features are preserved and it is important that some imperfections should remain to avoid a more feminised appearance.  This gentleman underwent an upper and lower lid blepharoplasty and mid face elevation.

The midface elevation has been extremely effective at creating a smoother at  the eyelid and cheek interface by elevating the drawn-down cheek. The swelling settled after 3-4 weeks giving a much more natural and less tired looking appearance.

Lower Lid Blepharoplasty

This gentleman presented with prominent lower lid bags caused by fat herniation, excess skin and muscle laxity. To address these concerns, an anterior approach lower lid blepharoplasty was performed to restore smoother lower eyelid contours and reduce under-eye fullness.

The result is a refreshed, natural-looking appearance that enhances the eye area while preserving the patient’s distinctive facial features. The postoperative photographs demonstrate a significant improvement in lower lid definition with scars that are exceptionally well concealed.

Bilateral Lower Lid Blepharoplasty Plus

This gentleman underwent a tissue-preservation lower lid blepharoplasty via an external approach, combined with fat repositioning and midface elevation. Rather than removing tissue, the fat responsible for the under-eye bags was carefully redistributed to restore volume in the hollow areas beneath the eyes.

The addition of a midface lift helped create a smoother transition between the lower eyelid and cheek, resulting in a more refreshed and natural appearance. The postoperative photographs were taken six weeks after surgery.

Blepharoplasty Plus With Direct Brow Lift

This patient presented with concerns about a tired and somewhat heavy appearance around the eyes, caused by a combination of brow descent, under-eye bags and midface ageing. To achieve the best possible outcome, an upper and lower lid blepharoplasty was combined with midface elevation and a direct brow lift. This comprehensive approach allowed us to address not only the eyelids themselves, but also the surrounding structures contributing to the overall appearance.

The procedures worked together to create a more open eye area, improve the transition between the lower eyelid and cheek, and restore facial harmony while maintaining the patient’s natural features. The postoperative photographs, taken three months after surgery, demonstrate a brighter, more refreshed appearance and highlight the benefits of our Blepharoplasty Plus approach.

Bilateral Upper Lid Blepharoplasty With Direct Brow Lift

This patient came to us with significant hooding and overhang in the outer corners of her upper eyelids. A blepharoplasty alone would not have been sufficient to fully clear the skin, so a brow lift was also performed. She opted for a direct brow lift, which involves an incision just above the eyebrow.

Six weeks post-operation, the results were reviewed, showing almost no visible scarring. The direct brow lift proved to be a very effective procedure, not only elevating the eyebrows but also lifting the crow’s feet area and the upper part of the cheek. This has opened the lateral part of the eye, resulting in a marked aesthetic improvement.

Bilateral Upper Blepharoplasty And Direct Brow Lift

This patient presented with significant sagging skin affecting the upper eyelids. However, a blepharoplasty alone would not have fully addressed the issue, as there was also notable brow descent contributing to the heaviness of the upper eyelid area. While there are various techniques available for brow lifting, the extent of this patient’s brow droop required a direct brow lift to achieve the necessary lift.

A small incision is made just above the brow hairline during this procedure, and it usually heals well. The patient opted for a combined upper lid blepharoplasty and direct brow lift, resulting in an excellent, natural-looking outcome that restored a more youthful and refreshed appearance.

Upper and Lower Eyelid Blepharoplasty with Ptosis correction and Midface Lift

This patient presented with numerous featured giving the eyes a tired appearance. The upper lid position was low and there was excess skin in both the upper and lower lids. This was associated with bulges and bags caused by fat herniation from the eye socket into the eyelid tissues.

The surgery lifted the upper eyelid position and removed the excess skin and fat. In addition, the midface lift has been highly effective in reversing the cheek descent and blending the lower lid with the cheek to give a more pleasing result. This example demonstrates how a midface lift can enhance the result of lower lid blepharoplasty by addressing cheek descent.

Upper Lid Blepharoplasty With Ptosis Correction, Internal Browpexy And Tear Trough Filler Injections

This patient presented with ptosis, excess upper lid skin, and a low brow position, all contributing to a tired appearance. Additionally, the lower lid tear trough added to the overall look of fatigue. Achieving maximum improvement required ptosis correction to lift the lid margin, blepharoplasty to remove the excess skin, and brow elevation to open up the eyelid area.

This combination successfully created a more open and refreshed eye configuration. Tear trough fillers further enhanced the result by addressing the hollows in the lower lids, providing a smoother and more rejuvenated appearance.

Lower Lid Blepharoplasty with Midface Lift and Fat Transfer

This lady presented with significant lower lid bags. The bags were due only in part to fat bulging. There was significant midface hollowing and descent of the cheeks. A full restorative approach was adopted to include lower lid blepharoplasty with midface elevation to restore the continuum between cheek and eyelid. Fat transfer was very effective at rejuvenating the cheeks and filling the hollowed areas which the blepharoplasty alone would not have been able to address.

This case demonstrates the importance of adjunctive ‘Blepharoplasty Plus’ procedures in achieving an optimal outcome. These pictures are taken 4 months after the initial surgery and the patient is delighted with the result.

Bilateral Lower Lid Blepharoplasty With MidFace Lift And Autologous Fat Transfer

This patient, in her early 30s, came to us concerned about a tired appearance around her eyes, caused by a combination of under-eye bags, significant hollowing, and cheek descent. She opted for lower lid blepharoplasty with midface elevation to address these issues, and also requested a more extensive fat transfer to enhance the lateral projection of her cheeks and reduce the prominence of her nasolabial fold, the deep groove running from the corner of the nose to the mouth.

The before and after images clearly illustrate how fat transfer can beautifully complement traditional blepharoplasty, delivering a dramatic improvement in her overall appearance. The result is a more youthful and refreshed look, achieved while maintaining her natural facial harmony.

Lower Blepharoplasty with Fat Transfer

This lady presented with lower lid bags and noticeable midface volume loss. Her case was technically complex due to a negative vector eye configuration, which increased the risk of lower lid retraction following blepharoplasty. To address these concerns safely and effectively, she underwent both upper and lower lid blepharoplasty, combined with a midface lift to provide essential support to the lower lids.

Additionally, fat transfer was performed to smooth and enhance the contour between the eyelid and cheek, delivering a beautifully natural result. The patient was delighted with her refreshed and rejuvenated appearance, which restored her natural vibrancy without altering her distinctive features.

Midface Lift and Blepharoplasty

Bilateral Upper and Lower Blepharoplasty with Midface Elevation

This patient presented with very significant hooding in the upper eyelids and large bags in the lower lids. She had noticed very significant changes to her appearance giving her a more tired and angry look. There were significant technical challenges with performing this surgery given that she has a negative vector eye configuration.

A decision was made to perform an upper and lower lid blepharoplasty with a direct brow lift to open up the upper eyelid space. A blepharoplasty alone would not have been adequate enough to achieve this. The lower lids were rejuvenated with a lower lid tissue preservation blepharoplasty and mid face lift.

Treatment with neuro modulation reduced the frown lines and reduced the dynamic lines around the skin areas. These photos demonstrate the result at the six week point after the surgery.

Eye Bag Blepharoplasty with MidFace Lift

This patient presented with significant lower lid bags, tear trough hollowing and midface descent, all of which contributed to a tired appearance. To address these concerns, a tissue-preservation lower lid blepharoplasty was combined with a midface lift. Rather than removing tissue, the fat responsible for the under-eye bags was carefully repositioned to restore volume and create a smoother transition between the eyelid and cheek.

Skin Treatments with Profhilo

This patient underwent an upper and lower lid blepharoplasty with internal browpexy to address upper eyelid heaviness and create a more open, refreshed eye area. The browpexy provided subtle support to the brow, enhancing the overall result while preserving the patient’s natural features.

To further optimise the outcome, Profhilo treatments were performed postoperatively to improve skin quality and soften fine lines. Together, these treatments created a refreshed and natural-looking result, as demonstrated in the postoperative photographs.

Upper and Lower Blepharoplasty with Midface Elevation and Internal Browpexy

This patient underwent an upper and lower eyelid blepharoplasty with midface elevation and internal browpexy using a tissue-preservation approach. Rather than removing significant amounts of tissue, the procedure focused on redistributing, tightening and elevating the existing structures to achieve a natural-looking result.

The lower lid bags were repositioned to restore volume in the hollow areas beneath the eyes, while the midface elevation created a smoother transition between the eyelid and cheek. A conservative upper blepharoplasty preserved the patient’s naturally hooded appearance, and the internal browpexy subtly elevated the tail of the brow to create a fresher, more open eye area. The postoperative photographs show the result 10 months after surgery.

Lower Lid Transconjunctival Blepharoplasty

This patient presented with significant lower lid bags and retained good skin elasticity, making her an ideal candidate for a transconjunctival lower eyelid blepharoplasty. This scarless approach allowed the under-eye bags to be treated without an external incision.

Rather than removing fat, the tissue was carefully repositioned to restore volume in the tear trough area and create smoother under-eye contours. The postoperative photographs demonstrate a refreshed appearance with a natural transition between the lower eyelid and cheek.

Revision Lower Lid Blepharoplasty and Midface Lift

This patient had previously undergone surgery elsewhere to address lower lid bags but remained unhappy with the outcome, as the bags persisted. A revision lower lid blepharoplasty was performed to correct the remaining under-eye fullness and restore a smoother lower eyelid contour.

To further enhance the result, a midface lift was carried out to reposition the cheek and improve the transition between the eyelid and cheek. The postoperative photographs demonstrate a more balanced, refreshed appearance with improved facial harmony.

Revision Surgery for Lower Lid Retraction and Scleral Show

This patient developed significant lower lid retraction following previous lower eyelid surgery performed elsewhere. The resulting scleral show, poor eyelid closure and altered appearance caused both functional and aesthetic concerns.

To restore lower eyelid support and position, revision surgery was performed using septo-retractor recession, cadaveric alloderm grafting and lateral canthoplasty. The postoperative photographs demonstrate a significant improvement in eyelid position, resulting in a more natural appearance and improved lower eyelid function.
